I acknowledge that there are risks associated with playing soccer during field related practices. In consideration for the privilege to use the field and attend training, my electronic signature indicates that I assume the risk of any injuries that my child may sustain while participating in any activity with Prep Soccer Training and will not hold liable the coaching staff or field representatives for any injuries that my child may sustain while on the premises of the outdoor or indoor playing location. I also certify that my son/daughter is physically fit to participate in all camp activities. Photo/Video Release: I understand that photos/videos may be taken and used for media purposes distributed and or exhibited only in conjunction with promotional activities pertaining to Prep Soccer Training program. I accept that no monetary compensation is or will be payable to me in the event that these photos are used. (The participant's name or identity will not be exposed.
